First Visit

What shall I bring for my first visit?

•Your current insurance card, any prescription/referral slips for therapy from your physician’s office.
•List of medications that you are currently taking.
•Completed patient registration, consent, and medical history form.

What should I wear?

•Loose comfortable clothing (ie: shorts, athletic pants, t-shirt).

Do I need a referral to come to you? And what is an “authorization?”

•Colorado is considered a “direct access” state where patients may refer themselves to Physical Therapy without a doctor’s referral (or prescription). However, your insurance policy may require a doctor’s referral or authorization.
•If your insurance policy is an HMO plan, the insurance company generally requires an authorization for you to see any specialist. Your “Primary Care Physician” (PCP) will often do the authorization for therapy.
•If you are unsure, please call us. We do provide a courtesy to you in checking your benefits prior to your therapy visit (If you would like us to check your benefits we would need your insurance information prior to your visit).

Do you bill my insurance?

•Yes, as a courtesy to our patients we will bill your primary insurance. If a balance is due from you we accept check, VISA, MasterCard, Discover, and Debit cards.

What if I don’t have any Health Insurance?

•If you do not have any Health Insurance, we can offer a self-pay discount. Please contact our office (970-460-0066 Johnstown/Loveland/Windsor or 970-568-8461 Fort Collins) for more information.

I completed a six week internship with RMPT, part of which I was with Rob in the Loveland office. Rob is an extremely knowledgeable, professional, and caring DPT. I thoroughly enjoyed my time learning the trade from him. I highly recommend this office for your physical therapy needs. -Chelsey, Loveland (Google Review 5stars)

I recently completed a six week internship with RMPT. I spent most of it with Amie and Jess in the Windsor clinic. They both are extremely knowledgeable, compassionate, and well rounded. I highly recommended using RMPT for all of your therapy needs. Chelsey, Windsor (Google Review 5stars)
