What shall I bring for my first visit?
•Your current insurance card, any prescription/referral slips for therapy from your physician’s office.
•List of medications that you are currently taking.
•Completed patient registration, consent, and medical history form.
What should I wear?
•Loose comfortable clothing (ie: shorts, athletic pants, t-shirt).
Do I need a referral to come to you? And what is an “authorization?”
•Colorado is considered a “direct access” state where patients may refer themselves to Physical Therapy without a doctor’s referral (or prescription). However, your insurance policy may require a doctor’s referral or authorization.
•If your insurance policy is an HMO plan, the insurance company generally requires an authorization for you to see any specialist. Your “Primary Care Physician” (PCP) will often do the authorization for therapy.
•If you are unsure, please call us. We do provide a courtesy to you in checking your benefits prior to your therapy visit (If you would like us to check your benefits we would need your insurance information prior to your visit).
Do you bill my insurance?
•Yes, as a courtesy to our patients we will bill your primary insurance. If a balance is due from you we accept check, VISA, MasterCard, Discover, and Debit cards.
